Telehealth General Consent for Care

1. Purpose of This Consent

This Telehealth General Consent (“Consent”) describes the nature of telehealth services offered by Tiger Medical, PLLC (“Tiger Medical,” “we,” “us,” or “the Practice”), the risks and benefits of receiving care in this manner, and the rights and responsibilities of patients who choose to receive telehealth care from Tiger Medical. By signing below, you acknowledge that you have read, understood, and agree to the terms of this Consent before receiving any telehealth services.

2. What Is Telehealth

Telehealth is the delivery of healthcare services using electronic communications, information technology, or other means between a licensed provider and a patient who are not in the same physical location. The Platform through which your care is delivered is a HIPAA-aligned, SOC 2-audited white-label telehealth technology solution owned and operated by an independent third-party company (the "Platform Operator"). Under a written Master Services Agreement between Tiger Medical and the Platform Operator, the Platform Operator provides the technology platform along with management, administrative, customer service, technology support, and fulfillment services that support Tiger Medical's delivery of clinical care. Through the Platform, Tiger Medical's licensed providers deliver both synchronous consults (real-time video, audio, or telephone visits) and asynchronous consults (structured intake, secure messaging, and provider review outside of real time). The Platform also supports appointment scheduling, secure messaging, electronic prescribing, laboratory ordering and results delivery, note charting, and payment processing. Tiger Medical is a cash-pay medical practice that does not bill health insurance and does not operate as a technical "covered entity" under HIPAA; the Platform Operator correspondingly does not act as a HIPAA "business associate" of Tiger Medical. Notwithstanding that technical status, Tiger Medical voluntarily maintains privacy and security practices consistent with HIPAA standards for all patients, and complies with the privacy, telehealth, and confidentiality-of-medical-information laws of every state in which it treats patients (including the California Confidentiality of Medical Information Act, the Washington My Health My Data Act, the Texas Medical Records Privacy Act, and comparable state statutes) (collectively, the "Privacy Laws"). You acknowledge that platform functions are provided by the Platform Operator under its own service terms, and that Tiger Medical is responsible only for the clinical services rendered by its providers through the Platform, not for the Platform's technical availability, performance, or administration. You understand that telehealth care through the Platform has technical limitations inherent to virtual care and to any third-party technology environment: audio or video quality may be affected by your internet connection or device; the Platform may experience scheduled maintenance or unexpected downtime; secure messages may be delivered with some delay depending on the consult modality; and in rare circumstances technical failures may prevent a scheduled visit from occurring. In the event of a technical failure, you agree to contact Tiger Medical using the contact information below or through the Platform's support channel to reschedule; in the event of a medical emergency, you agree to seek in-person emergency care and not to rely on the Platform. Your provider will determine, using their clinical judgment, whether your presenting concern and requested treatment can be appropriately addressed through async consultation, or whether a synchronous visit is required.

Asynchronous (“Async”) Consults: You submit health information, intake forms, photographs, and/or messages through the Platform, and a licensed Tiger Medical provider reviews this information and responds at a later time. There is no live, real-time interaction between you and the provider during an async consult.

Synchronous (“Sync”) Consults: You and a licensed Tiger Medical provider communicate in real time using live video, audio, or telephone technology made available through the Platform.

3. Services Offered

Tiger Medical provides telehealth services that may include, without limitation:

Hormone Replacement Therapy (HRT)

Peptide therapy

GLP-1 receptor agonist therapy, in both oral and injectable formulations

Related laboratory review, monitoring, and follow-up care

Not every service is appropriate for every patient. Your provider will determine, based on your intake information, history, and (where applicable) laboratory results, whether a given treatment is clinically appropriate for you. Tiger Medical reserves the right to decline to treat, or to discontinue treating, any patient for whom telehealth care or a requested therapy is not clinically appropriate.

4. Compounded, Off-Label, and Non-FDA-Approved Medications

You understand and specifically acknowledge the following regarding medications that may be prescribed to you through Tiger Medical:

Compounded Medications: Some medications prescribed may be compounded by a licensed pharmacy according to your provider's prescription. Compounded medications are prepared for an individual patient and are not reviewed by the U.S. Food and Drug Administration (“FDA”) for safety, efficacy, or quality in the same manner as FDA-approved manufactured drug products.

Off-Label Use: Some medications may be prescribed for a use, dosage, patient population, or route of administration that has not been specifically approved by the FDA (“off-label” use). Off-label prescribing is a legal and common practice in medicine, and your provider will use their clinical judgment in recommending such treatment.

Non-FDA-Approved Medications: Certain peptides and other agents offered by Tiger Medical may not have any FDA-approved indication and may be classified as investigational, research-use, or otherwise lacking FDA approval for human therapeutic use.

You acknowledge that the risks, benefits, and long-term effects of compounded, off-label, and non-FDA-approved medications may not be as well established as those of FDA-approved, on-label medications, and you voluntarily assume these risks by choosing to proceed with treatment.

5. Benefits and Risks of Telehealth

Potential benefits of telehealth include increased access to care, convenience, reduced travel and wait times, and continuity of care. Potential risks and limitations include, without limitation:

Information transmitted electronically may, in rare instances, be intercepted, disrupted, corrupted, or accessed by unauthorized parties despite reasonable security safeguards.

Technical failures (e.g., poor internet connection, dropped calls, or outages of the Platform, which is owned and operated by a third party) may delay or interrupt care.

The absence of an in-person, hands-on physical examination may limit your provider's ability to identify certain conditions that would otherwise be detected in person.

In rare cases, your provider may determine that your condition cannot be safely or adequately evaluated or treated through telehealth and may recommend in-person evaluation, laboratory testing, or a referral to another provider or facility, including emergency care.

Async consults, by their nature, do not permit real-time clarifying dialogue; you are responsible for providing complete and accurate information in your intake.

6. Not for Emergencies

Telehealth services offered by Tiger Medical are not intended for medical emergencies. If you are experiencing a medical emergency, or believe you may be experiencing a life-threatening condition, call 911 or go to the nearest emergency room immediately. Tiger Medical is not able to guarantee immediate response times and does not provide emergency or crisis services.

7. Patient Responsibilities

As a condition of receiving telehealth services, you agree to:

Provide complete, accurate, and truthful information regarding your identity, location, medical history, current medications, and symptoms.

Confirm that you are physically located in a state in which Tiger Medical and its providers are authorized to deliver care at the time of each consult.

Inform your primary care provider (“PCP”) and any other treating medical providers of all medications, including any HRT, peptides, GLP-1 medications, or other treatments, prescribed to you by Tiger Medical. You understand and acknowledge that Tiger Medical is not your PCP, does not serve as a substitute for primary care, and does not coordinate your care with your PCP or other providers on your behalf — you are solely responsible for this communication.

Seek immediate in-person evaluation at a local urgent care clinic or emergency room if you experience any adverse reaction, side effect, or concerning change in your condition following treatment prescribed by Tiger Medical. Because Tiger Medical operates on a telehealth basis, contacting your Tiger Medical provider first may delay evaluation and treatment; you should not wait for a response from Tiger Medical before seeking in-person care if your symptoms warrant prompt attention. You may separately notify Tiger Medical of the reaction once you have sought appropriate in-person care. You assume the risk of any delay in treatment resulting from a failure to seek prompt in-person evaluation.

Follow up as recommended by your provider, including any requested laboratory work or monitoring.

Understand that failure to provide accurate information may affect the safety and appropriateness of any treatment recommended.

8. Privacy and Confidentiality

Tiger Medical is a cash-pay telehealth medical group that does not bill health insurance and does not operate as a technical "covered entity" under the Health Insurance Portability and Accountability Act ("HIPAA"); accordingly, HIPAA does not directly govern Tiger Medical's clinical services. Tiger Medical nonetheless voluntarily maintains privacy and security practices consistent with HIPAA standards for all patients, and complies with the privacy, telehealth, consumer-privacy, and confidentiality-of-medical-information laws of every state in which it treats patients, including but not limited to the California Confidentiality of Medical Information Act and California Consumer Privacy Rights Act, the Washington My Health My Data Act, the Texas Medical Records Privacy Act, and the consumer privacy statutes of Nevada, Colorado, Connecticut, Virginia, and other states. Please refer to our Privacy Notice, provided separately, for a full description of how your information may be used and disclosed and your rights with respect to that information.

9. Scope of Our Role; Third-Party Vendors and Storefronts

Tiger Medical is a medical provider company. Our role, and the role of our providers, is limited exclusively to clinical evaluation, medical recommendations, and prescribing. Tiger Medical does not perform advertising or marketing, does not process payments, does not issue refunds, does not manage patient accounts or billing, and does not perform collections. Tiger Medical does not own, operate, or control — and is not responsible for — the following:

Advertising and Storefronts: any advertising, marketing claims, pricing, or promotional content published by a third-party storefront, marketplace, affiliate, or reseller through which you may have learned of or accessed our services;

Payments, Billing, Refunds, and Collections: payment processing, invoicing, billing, refunds, chargebacks, or collections activity of any kind. These functions are performed exclusively by the applicable payment processor, platform operator, or other third-party vendor — never by Tiger Medical;

Pharmacy and Shipping: the pharmacy that fills your prescription, or that pharmacy's inventory, compounding process, packaging, shipping carrier, or delivery timeline; and

Platform, EMR, and Support: the technology platform, electronic medical record (EMR) system, patient account and billing management tools, or non-clinical support team that may facilitate your access to care, which may be operated by independent third parties.

Transition of Care by Third-Party Vendors: any decision by a storefront, marketplace, platform operator, or other third-party vendor through which you accessed our services to change its provider network, transition your care to a different medical group, or end its relationship with Tiger Medical. Tiger Medical does not control such decisions and cannot guarantee continuity of care through that vendor. If this occurs, Tiger Medical may no longer be authorized or able to continue treating you through that platform, and you may need to establish care with the new provider group or elsewhere.

Any refund request, billing dispute, chargeback, collections matter, or account management issue must be submitted to and resolved directly with the applicable payment processor, platform operator, or other responsible vendor. Tiger Medical does not determine, approve, or issue refunds and has no authority over a vendor's billing, account, or collections decisions. By signing this Consent, you agree that any concern relating to the items above is outside the scope of the medical care provided by Tiger Medical and should be directed to the appropriate third party.

10. Reviews, Feedback, and Regulatory Complaints

We take patient concerns seriously and encourage you to raise them with us directly at notices@lion.md or (561) 652-5360 so we can address them.

Tiger Medical does, however, reserve the right to pursue all available legal remedies, including a claim for defamation, against any individual who knowingly publishes false statements about Tiger Medical or its providers with actual malice — that is, with knowledge that the statements are false or with reckless disregard for their truth. This provision does not limit or penalize truthful statements, good-faith reviews, or good-faith complaints made to a regulatory or licensing authority.

11. Assumption of Risk and Release of Liability

By signing this Consent, you voluntarily choose to receive care through telehealth and acknowledge that you have had the opportunity to ask questions about the nature of telehealth, the services offered, and the medications described above. In consideration of being permitted to receive telehealth services from Tiger Medical, you, on behalf of yourself, your heirs, and your legal representatives, agree to release, waive, and discharge Tiger Medical, PLLC, its owners, providers, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to your receipt of telehealth services, to the fullest extent permitted by applicable law. This release does not apply to claims arising from gross negligence, willful misconduct, or as otherwise prohibited by law.

12. Right to Withdraw Consent

Your consent to telehealth services is voluntary. You may withdraw this Consent at any time by notifying Tiger Medical in writing, without affecting your right to future care in another appropriate manner. Withdrawal of consent may result in the discontinuation of certain ongoing treatments, and you understand that abruptly stopping some medications (including hormone therapies) without medical guidance may carry its own risks, which your provider can discuss with you.

13. Governing Law

This Consent shall be governed by the laws of the State of Florida, without regard to its conflict-of-laws principles, and by applicable federal law.

14. Acknowledgment and Signature

I have read this Telehealth General Consent, or have had it read to me, in its entirety. I understand its contents, including the description of async and sync consults, the use of compounded, off-label, and non-FDA-approved medications, my responsibility to inform my PCP and other treating providers of medications prescribed by Tiger Medical, my responsibility to seek prompt in-person care at a local urgent care or emergency room for any adverse reaction, the limited scope of Tiger Medical's role relative to third-party pharmacies, platforms, and storefronts, and the release of liability described above. I have had the opportunity to ask questions, and all of my questions have been answered to my satisfaction. I voluntarily consent to receive telehealth services from Tiger Medical, PLLC.
